Formly

Meet Formly

Formly creates forms, surveys, and quizzes from a description of what someone wants to learn or collect. Its AI generates questions and branching logic, while styling controls help the finished form match a brand. Templates cover uses such as lead qualification, customer satisfaction, and personality quizzes.

Responses appear in a dashboard, with charts that can also show respondents their results after submission. Partial response capture saves answers as they are entered, helping teams see unfinished submissions. Notifications and Google Sheets integrations connect collected information with follow-up workflows. Formly suits businesses, educators, and creators who need interactive questionnaires without building a custom application.
